3/8/2024 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Jeremiah arrived to session appearing to be in a calm mood by greeting QMHP-C and appeared to be well-groomed. Jeremiah was struggling with focusing to the instructions as evidenced by Jeremiah playing on his phone and influenced by his surroundings at which this prohibits him from being able to focus within the home setting at least 2- 4 times weekly with behaviors lasted up to 3 minutes in duration. 2. Behavior: Jeremiah presented withdrawn behaviors as he gets upset with his mother and his mother boyfriend when he cannot have his way at least 4 times weekly at which behaviors lasted up to 2 minutes in duration which also consist of him overtalking others or not following directives the first time they are given. 1.Intervention: QMHP-C greeted Jeremiah and redirected him to focus and put his cell phone away. QMHP-C influenced Jeremiah be accountable for his actions and identify his self-images. QMHP-C provided role playing activities to help the interview himself. QMHP-C help Jeremiah with defining self-images and how to create positive images about himself. QMHP-C provided awareness wheel for Jeremiah to help identify himself. 2. Intervention: QMHP-C assisted Jeremiah with self-help guide to overcoming symptoms of stress, anxiety and low. QMHP-C provided several worksheet to help Jeremiah with identify himself and understand the characteristic of himself and see if his friends to identify himself. 1.Response: Jeremiah greeted QMHP-C appropriately by using appropriate tone and being respectful by showing eye contact. Jeremiah responded aggressively when asked to put his phone way. Jeremiah was struggling with completing his activities as evidenced by not responding about himself. 2. Response: Jeremiah was receptive after several redirections as evidenced by identifying his emotions and be aware of his self-awareness by making positive choices.
